VETERINARIAN AUTHORIZATION
During my absence, a representative of PETS PANTRY will be caring for my animal's welfare and has my permission to transport them to a Veterinary Surgery for consultation and treatment if necessary. I will be fully responsible for payment upon my return for any veterinary treatment.

We use the East Neuk Veterinary practice in St Monans , Fife
